I would use Dr. Afaf Meleis theory on transition
theory to support
my concept analysis around motherhood transition. Dr. Meleis have used transitions
theory to study many types of transitions such as the immigration transition,
the transition of caregivers, the transition of patients with cardiovascular
events, the transition to motherhood, and the menopausal transition, as well as
the transition of women toward integrating their roles while enhancing their
wellbeing. 

 

In
my research, I will explore the early skin at the postpartum period and it’s
benefit on the baby and the mother. The motherhood transition and maternal-
newborn bonding are directly related. Motherhood transition which is a key
component to promote nurse role in the maternal-newborn bonding. Moreover, Dr. Meleis theory
is focused on how nurses can make a difference in helping people achieve
healthy outcomes after their transitions.

Using the transition
theory in motherhood can give the new mother an optimal outcome after giving
birth. Motherhood may be described as a process of role transition that
begins during pregnancy. The transition ends when the mother develops a sense
of comfort and confidence in performing the parental role.

The Understanding of the transition
and how to help the new mother to go through this new experience, it would be
very helpful for the nurses to promote the maternal newborn bonding.

The use of the theory is very
useful in the postpartum period, because is facilitate a healthy transition by assuming
her new role.

Some may critique the uselfness of theory in nursing interventions especially
in motherhood transition since it’s a natural bonding from a child and mother
perspective. For some mothers bonding is natural and won’t need any nurses interventions,
while other mothers may need advance nursing interventions.

Using Dr. Meleis transition theory
that she called for knowledge development in nursing to be about nursing
therapeutics rather than understand the phenomena related to responses to
health situations. The therapeutic approach that the nurse need to intgrate in
the patient care is the most important in helping the new mother in
transitioning in motherhood and engage her bonding with her newborn.

The article that I choose for the
purpose of this paper is “Perceived Readiness for Hospital Discharge in Adult
Medical-Surgical Patients “(waines,2017). the authors used a correlational,
prospective, longitudinal design with path analyses was used to explore relationships
among transition theory to related variables. The study is using Dr. Meleis
transition theory in conducting this study to understand how the variables are
impacting the patients in the discharge transition.

The theory was used to guide
identification of variables for this study. The purpose of this study was to
identify patient characteristics, hospitalization factors, and hospital nursing
practices that are predictive of adult medical-surgical patients’ perceptions
of their readiness to go home at the time of discharge and the relationship of
perceptions of discharge readiness to post hospitalization coping and
utilization outcomes

 Hospital discharge was viewed as a
transitional process occurring in three phases: (a) the hospitalization phase
during which discharge preparation occurs, (b) the discharge when short-term
outcomes of the preparatory process can be measured, and (c) the post-discharge
period when patients’.

Four major dimensions of
transitions theory were explored in this study: the nature of the transition
(hospitalization factors including planned or prior admissions and length of
hospital stay), transition conditions (patient characteristics including age,
gender, race, socioeconomic status, and living alone), nursing therapeutics
(discharge teaching and care coordination), and patterns of response (readiness
for hospital discharge, post discharge coping difficulty, and post discharge
utilization of health services). Transitions theory proposes that the nature of
the transition, transition conditions, and nursing therapeutic practices will
affect patterns of response during a transition. Transitions theory concept
definitions and specification of the related study variables and empirical
indicators.

On the article the method used to
measure the variable concerning the nursing practice prior discharge was not
clarified. The method could be more in depth of the nursing care perceived and
the quality of patient teaching prior to discharge and how this variable played
a major role on the outcomes for the discharge transition.

Finally, the study results  support Meleis’ transitions theory as a
useful model for conceptualizing and investigating  framework to the discharge transition.
Moreover the results have implications for the clinical nurse specialist role
in patient and staff education, system building for the post discharge
